A free physical event will be held at the Smith County Wellness Building (behind Smith County Middle School) on the following dates:

 Grades K–7:
• Monday (5/4) & Tuesday (5/5)
• 3:00–5:30 PM

 Grades 8–11:
• Wednesday (5/6) & Thursday (5/7)
• 3:00–5:30 PM

 Important Reminders:
• Please print and complete all paperwork before arriving to speed up the process.
• If your school uses Final Forms (SCHS, SCMS, GHS), all documents are completed online.
• Be sure to click the blue “26–27 REGISTRATION” link at the top.
• After signing all required pages, go to the “TSSAA PPE Physical” tab, click print, and bring it with you.

 If you don’t use Final Forms, or are unsure, bring the completed physical forms for your student’s grade level.

 The more completed and signed paperwork you bring, the faster your visit will be!
